SERVICES - HOME BASED
With a focus on independence and integration into everyday community
life, EduCare of Texas provides Home Based Services funded and managed
through the Community Living and Support Services (CLASS), Home and
Community-based Services (HCS), and Texas Home Living (TxHmL) programs.
These programs provide opportunities for individuals to remain in their
own homes with family members or roommates and receive many of the supports
that they may require.
Individuals and their families or guardians decide which types of supports
are needed and identify available funding. In many cases with the assistance
of a case manager, the needed services are integrated into a Person-Directed
Plan. The Plan not only specifies the waiver services a participant
receives, it also identifies the non-waiver services that the individual
accesses in the community. Services can include: Occupational Therapy,
Physical Therapy, Psychology, Speech Therapy, Respite, Day Programming,
Residential Supports, Nursing, Dental Services, Adaptive Aids, and Home
Modifications, see below for more details. Additionally, in the CLASS
program, Specialized Therapies may include: music therapy, recreation,
massage therapy, and hippo-therapy, see below for more details.
Assistance with Personal Care
Anyone who has ever had to rely on assistance from others to meet their
personal care needs knows how absolutely critical those services can
be. EduCare of Texas' caregivers are dedicated to providing this very
important service, skillfully and respectfully. We understand the importance
to and the impact on the person receiving the service. We understand
that people providing personal care must have the training to do the
job correctly, but also must care deeply about the service they provide.
Our personal-care staff have the knowledge and understanding to provide
that day-to-day, hour-by-hour care in the client’s own home, with
respect for their privacy and personal preferences.
Nursing Services
If you have a chronic medical condition and want to maintain the best
possible health, one of the most effective ways to do so is through
consistent monitoring by a qualified, well-trained professional. Our
nurses are all of that and more! We provide a full array of services
to assist individuals in achieving and maintaining their goal of optimum
physical health. Comprehensive evaluations, consistent monitoring of
health conditions, referrals for appropriate medical services, assistance
with medications, physician-ordered treatments, as well as thorough
training for the individual and their family members are just a few
of the services our nurses can provide.
In short, our nurses are trained to give you the best care available
in a home setting and refer you for further treatment, when appropriate.
At EduCare of Texas, we know that most individuals are their happiest
and healthiest when they are in an environment in which they feel comfortable
and supported. We strive to provide all the care necessary to allow
our patients to continue to live in their home environment with privacy,
dignity and good health!
Occupational, Physical and Speech Therapy
At EduCare of Texas, our experienced, professional therapists are ready
to assist you or your family member with the goals of obtaining greater
independence and improved functioning through a variety of therapeutic
modalities. Such assistance includes: assessment, development of a therapeutic
treatment plan, direct therapy, training and assistance with adaptive
aids and communication devices, as well as consultation with other service
providers and family members.
Our therapists also participate as members of your treatment team, working
in coordination with other team members to provide you with the most
comprehensive and consistent care. This team approach means that the
services you receive build on each other and enhance the overall service
plan.
Specialized Therapies
There are some needs that go beyond the standard array of therapeutic
options, and EduCare of Texas is pleased to offer these services that
meet many of those needs. There are a number of therapy options and
services that fall into this category including: recreational therapy,
music therapy, massage therapy, hippo-therapy, hydrotherapy, therapeutic
horseback riding, auditory integration/enhancement training (AIT-AET),
aquatic therapy, and nutritional services.
Our therapists and instructors are licensed and/or certified as appropriate.
Their extensive knowledge and experience in their field mean that you,
our client, receive highly professional services, with rewarding therapy
benefits.
Psychological Services
We all know that every once in a while you hit a bump in the road of
life. Our team of psychologists stands ready to assist you and/or your
loved one with any emotional and psychological challenges.
Life can be stressful enough for all of us, even without the existence
of physical and mental disabilities that can compound emotional problems.
EduCare recognizes this by providing a full array of psychological services
to meet our client’s needs, including assessments and counseling
with consumers and families. In addition, we address behavioral concerns
by developing individualized behavior plans that are designed to teach
adaptive behaviors, and by assisting family and friends to respond appropriately
to behavioral issues, as well. EduCare's focus on individualized treatment
and the development of positive, adaptive behaviors has proven successful
for those looking for answers to complicated issues.
